To submit an information request, complete the REQUESTING PARTY INFORMATION and CALL INFORMATION REQUESTED sections of the form below with as much information as possible.
Please include type of incident (wreck, battery, fire, etc.), date and time of incident, and location of incident. Indicate the type of information desired ( copy of phone call , printed copy of call information, etc.)
The requesting party will be contacted regarding fees involved. Any required fees must be paid before copies of the information can be delivered. Examination of the information, without copies being provided, may be done at:

During business hours (weekdays between the hours of 8:00am – 12:00pm and 1:00pm – 5:00pm). Fees will not be charged for examination of public records during normal business hours.
NOTE: Per Louisiana Revised Statute 44:3, any incident for which there is a pending criminal court case or for which a criminal court case is not public record. It can only be obtained through court order or subpoena. The District address contact information is:
